A prequalification questionnaire (PQQ) is the formal way to assess whether a subcontractor meets your minimum standards before you appoint them. It covers company information, insurance, competence, health and safety track record, and references — everything you need to make an informed decision under CDM 2015.
What this tool does
This template provides a structured PQQ covering six sections: company information, insurance and financial standing, competence and qualifications, health and safety, environmental practices, and references. Fill it in online to create a pre-populated PDF, or download a blank template to send directly to subcontractors. The questionnaire is based on industry-standard PQQ structures adapted for UK construction SMEs.
Why prequalification matters
CDM 2015 (Regulation 8) requires you to take reasonable steps to satisfy yourself that anyone you appoint has the right competence. A structured PQQ gives you documented evidence of that assessment. It also filters out subcontractors who can't meet basic requirements before they reach your site — saving time, reducing risk, and providing an audit trail for client reviews.
Adapting the template
This template covers the essentials. Depending on your project, you may need to add client-specific requirements (approved supplier lists, framework conditions), project-specific risks (working at height, confined spaces, asbestos), or sector-specific standards. The downloadable PDF can be edited to add your company logo and additional questions.
This tool is for guidance only and does not constitute legal advice. Requirements vary by project scope and applicable regulations.
